1. Medical Documentation Required

  • Order for ABA Services
  • Letter of Medical Necessity
  • Previous Medical documentation from Pediatrician, Neurologist, IEP from Public School

2. Comprehensive Diagnostic Evaluation

  • CDE is required. Intake coordinator can coordinate with Psychologist for CDE
  • Psychologist will reach out to schedule evaluation date

3. Insurance Approval Process

  • Once CDE is received, we await insurance approval
  • Once insurance approves, the intake coordinator will let you know as well as collect the following information:
  • Location of services
  • Schedule for services
  • A Board Certified Behavioral Analyst will contact you to schedule a date for assessment
  • Two questionnaires will be sent to your email and must be completed prior to evaluation date
  • Assessment will be sent for parent reviewal and signature
  • Once signed, it will be sent to the insurance for approval of hours

How Many Hours per Week are Approved?

An Average of 25-30 hours.

Approval depends solely on child’s need and severity of diagnosis
